About Us

Southern Grace Hospice is a community-based program serving Morgan County and four surrounding rural communities, Greene, Putnam, Hancock and Oconee Counties, offering complete hospice services. Our interdisciplinary team works closely with families to provide personalized care to their loved one, primarily in the home. The Hospice services provided include physician services, home care nursing, social work, spiritual care, bereavement counseling, and the assistance of trained volunteers. The Hospice team works in cooperation with the patient’s personal physician to provide compassionate professional care and loving support for the patient and their families.
