Southern Grace Hospice is considered to be the model for quality, compassionate care for people facing a life-limiting illness or injury.
Our hospice care involves a team-oriented approach to expert medical care, pain management, and emotional and spiritual support expressly tailored to the patient’s needs and wishes.
Hospice care neither prolongs life nor hastens death. At the center of hospice and palliative care is the belief that each of us has the right to die pain-free and with dignity, and that our families will receive the necessary support to allow us to do so.
Our collaborative team of care providers is here to help people with serious illness live life fully, maintain their dignity, and ensure his or her quality of life is the best it can possibly be.
Who uses Hospice care?
Hospice is available to anyone, from children to elderly adults, whose life expectancy is measured in months, rather than years. We care for patients with many different diagnoses like cancer, dementia, cardiac disease, renal failure, liver disease, multiple sclerosis, Parkinson’s, and HIV.
How can Hospice help?
Southern Grace Hospice is here to ease the burden of caring for your loved one. Our goal is for you to be able to focus on being a family member instead of a caregiver. Our team is made up of exemplary healthcare professionals including physicians, nurses, CNAs, social workers, chaplains, counselors, and volunteers. We work closely with you and your family to develop an individualized plan of care tiered specifically to you and your needs.
About Us
Our team members do not view the care we provide as work, but rather a ministry in which we can serve and better the lives of those around us. At Southern Grace, we don’t focus on death and dying, but rather on life and living.
